Oxford is introducing 2 new HMO plans coming July 1st to small companies with 2-50 employees. The new plans offer coverage through their "Liberty" Network and are priced at about $400.00 per individual.
The new plans are "in-network" only and feature:
- No charge for routine well visits
- $30 copays for doctor visits and $50 copays for specialist visits
- Gym benefit that reimburses up to $200 per 6 month's of membership
- Three tier (generic, brand and formulary) RX card for prescriptions
